TEAM
CHECK IN
Dear
Coach or Manager,
Teams can check in during the week of Oct 9-11 at
our league office during normal business hours, Tue and Wed 10 am to 3 pm, Thur
3 to 6 pm.
All other check in will be at the shelter house at
our field complex at least 1 hour prior to your first game.
If your team has a Friday night game and you do not check in
during the week, you can check in at the shelter house at our field complex
before your game.
All
teams will be required to supply the following items at check in -

2007-2008
OFFICIAL STATE ROSTER (copy
for our files)
· 2007-2008 PLAYER & COACHES ID CARDS, laminated with pictures
· GUEST PLAYER FORM, if needed, (copy for our files)
· TRAVEL PERMIT, for out of Missouri teams (copy for our files)
· NOTARIZED MEDICAL RELEASE FORM for all players
A
guest player form and a medical release form are on our FORMS page, if needed. If you already have a similar
form that is fine, use the one you have.
