GENERAL REGISTRATION PROCEDURES

NOTE:

If you desire a copy of any paper work, make copies BEFORE you turn it in.

PRINT clearly on all forms.

Make sure all of your paperwork is complete and in order. If there is missing information or paperwork, the packet will be returned unfinished.

Player Registration

	Fill out MEMBERSHIP FORM
	Copy of Birth Certificate or approved Proof of Age documents (see US Youth Soccer Policy Rule 204). Hospital, baptismal or religious certificates will not be accepted
	A player must register in the state in which they reside, regardless of where their team plays
	There is a $15 yearly fee

Coach/Manager Registration

	Fill out MEMBERSHIP FORM
	Copy of Coach License
	Every coach must submit a KIDSAFE clearance form each seasonal year. Go to state web page to fill out form online and print off a hard copy of your CONFIRMATION PAGE. MYSA KID SAFE
	There is a $15 yearly fee

Player Pass (Cards)

	Both Players and Coaches are required to have to be on team sideline. It is proof of current registration and eligibility
	Current wallet size photo attached to back
	Signature on back must match name on front and on roster. If participating in State Cup, signature must match EXACTLY.
	Must be permanently laminated. Laminating supplies are available at no charge at the WMSL league office
	Will be generated at the WMSL league office and returned to the team to attach photo, be signed and laminated

Team Registration

	Fill out TEAM SIGNUP FORM
	All teams should have a distinct name. Even though teams are different ages, identical names can be confusing. Distinct team names will be assigned by the league if necessary.
	Fill out ROSTER WORKSHEET and attach all membership forms, birth certificates, coach license, Kidsafe copy or Disclosure statement
	An official MYSA Team Roster will be generated and returned to the team. Competitive teams must have a copy of their official MYSA roster at game sites at all times.
	Teams playing any games, league or tournament, outside of Missouri must complete an APPLICATION TO TRAVEL FORM

Adding a Player

	Only players that have not been rostered on another team during this seasonal year can be added
	Fill out ROSTER CHANGE FORM, showing player as “A” for add
	Fill out MEMBERSHIP FORM
	Copy of Birth Certificate
	$15 yearly registration fee

Transfer Player

	Player must be dropped from previous team before they can be transferred
	A player’s request to transfer must be allowed unless the player has an unfulfilled financial obligation to the team
	Fill out ROSTER CHANGE FORM, show player as “PT” for primary transfer, or "ST" as secondary transfer
	Fill out MEMBERSHIP FORM
	Copy of Birth Certificate
	Any change of status (form Primary to Secondary, etc.) is considered a transfer
	The team gaining the player is charged with a transfer, not the team losing the player
	There is an additional $15 yearly fee

Dropping a Player

	Fill out ROSTER CHANGE FORM
	A player’s request to drop must be allowed unless the player has an unfulfilled financial obligation to the team
	The player ID card must be returned

Dual Roster Player

	The recommended maximum number of dual rostered players on a team is 3
	Player must fill out MEMBERSHIP FORM and supply copy of Birth Certificate for both teams
	Fill out DUAL ROSTER REQUEST FORM
	If adding player after initial registration, fill out ROSTER CHANGE FORM and show player as an add
	There is an additional $15 yearly fee

Recruiting

During the seasonal year no coach, player, parent, or any other team member may approach a rostered member on two competitive teams for the purpose of asking them to tryout and/or play for that team, in that year, or any subsequent year. Exception: During the period of tryout start date (June 15) to August 31 of the current seasonal year, players may be recruited for the upcoming season.
