Game Reschedule Request Form

 

Please fill this form out COMPLETELY. Incomplete information may cause delay and game not to be rescheduled.

Fill out this form for games to be played at WMSL fields ONLY.

If game is at some other field, that home league must be contacted.

1) Contact the other coach involved and get him to agree to the change.

2) Come up with 3 dates for the reschedule that works for you and the other coach.

3) If the requesting coach does the work of contacting the league and the other coach and coming up with reschedule dates there will be no charge. If the League must get involved and do the contacting and rescheduling there will be a $50 fee, payable before the reschedule happens.

4) This form and both coaches agreeing to the change must be into WMSL league office BEFORE noon on Monday of the week the game was originally scheduled.

5) If you notify the league to change your game after the above deadline there will be a fee assessed to the requesting team of the referee's pay plus referee assignors pay ($14 to $76 depending on age of game).

6) WMSL scheduler, after receiving this form, will make the change and inform all parties concerned VIA EMAIL.
