Fill out this form for games to be played at WMSL FIELDS ONLY
If game is at some other location, contact that home league to reschedule.
Procedure
1) Contact the other coach involved and have him agree to change
2) Come up with 3 dates for the reschedule that works for both teams
3) If the requesting coach does the work of contacting the other coach and coming up with reschedule dates there will be no charge. If the League must get involved and do the contacting there will be a $50 fee, payable before the reschedule happens
4) The non-requesting coach "signs" his approval of the change by either emailing the league at wmslsched@kc.rr.com or calling the league office and leaving a message at 816-459-7543. No action will be taken on the request until the league office receives this notification from other coach.
5) This form and both coaches agreeing to the change must be in to WMSL league office BEFORE noon on Tuesday of the week the game was originally scheduled. If received after this time, a fee will be assessed to the requesting team of the referee's pay plus referee assignor's fee ($14 to $76, depending on age of game).
6) WMSL scheduler will make the change and inform both teams VIA EMAIL.
